FIGURE 1 A diagrammatic representation of the subdivisions of human substantia nigra as defined by Hassler. The shaded subnuclei in the ventral and lateral regions degenerate selectively in the early stages of PD. Modern molecular biological approaches are pointing to the possibility that dysfunction of a variety of cellular mechanisms may result in an insidious and a slowly progressive levodopa-responsive parkinsonism that is indistinguishable from PD. These observations raise the issue of whether there may be multiple etiologies for PD. In this chapter, some of the neurochemical changes noted in the degenerating dopaminergic neurons of the substantia nigra in experimental models of PD, inherited forms of PD, and idiopathic PD will be summarized. GROWTH FACTORS AND THE SUBSTANTIA NIGRA Neurons and glia need a very small concentration of trophic or growth factor(s) for their continued existence, maintenance of a normal connectivity and physiological state, as well as recovery from chemical and physical injury (6). If these growth factors are withdrawn or if their continuing influence is affected both neurons and glia may fail to differentiate and die during embryogenesis. As adult neurons, they may lose their efficiency or Copyright 2003 by Marcel Dekker, Inc. Under stressful circumstances, neurons and glia upregulate the expression of growth factors and their specific receptors or acquire them from their target neurons and the surrounding glial cells and recover from the injury. Age-dependent decline and withdrawal of neurotrophins and their receptors (7,8) are thought to play an important role in the pathogenesis of Alzheimer’s disease. The dopaminergic neurons of the substantia nigra are dependant on several such trophic and growth factors for normal ontogenesis and survival as fully differentiated mature adult neurons.

In those studies in which DSM-III or DSM-III- R criteria for depression were strictly followed discount trileptal 600 mg with mastercard, the prevalence was higher at 42. Dysthymic disorder is even more common, prompting some to suggest that this might be an intrinsic property of PD itself (27). Pathophysiology The depression of PD is said to occur in a bimodal distribution, with most depressed patients having either early or advanced disease (28). Those with moderate symptoms of PD have less depression. This observation fits the notion that depression in PD is a reactive depression, which develops when patients find out that they have a degenerative brain disease at the time of diagnosis and again late in the course when disease symptoms are worse. A study comparing the frequency of depression in PD and arthritis showed no difference, which also supports this reactive depression model (29). However, other studies have shown a higher incidence of depression compared to other chronic disabling diseases, calling the reactive depression hypothesis into question (30). The alternative view is that the neurodegenerative process in PD is the proximate cause of depression through depletion of brain monoamines. Support for this view is found in studies showing that depression is more common among those PD patients with a significant decline in cognitive function in a 12-month period (31) and those studies demonstrating loss of dopaminergic neurons in the ventral tegmental area in patients with parkinsonism, dementia, and depression (32). However, reduced CSF 5HIAA was also seen in some PD patients without depression, indicating that this biochemical change is not sufficient to cause depression but might create a susceptibility to depressive illness. Clinical Features Recognizing the clinical features of depression in PD patients is a challenge, mainly because several key features of depression, such as loss of appetite, concentration difficulties, sleep disturbances, and slowness of movement, are features of PD itself (35). A recent study showed that during the course of routine follow-up visits for PD, neurologists with special expertise in movement disorders correctly made the diagnosis of depression in only 35% of patients who were known to have depressed mood, as shown by a Beck Depression Inventory score (BDI) greater than 10 (36). The routine use of Copyright 2003 by Marcel Dekker, Inc. PD related depression presents with dysphoria characterized by the presence of hopelessness, pessimism, and decreased motivation. Negative features such as guilt and feelings of worthlessness are not often seen (29).

BDNF buy trileptal 300mg cheap, when injected into the striatum, is transported only to the soma of dopaminergic neurons of the substantia nigra. These findings suggest that BDNF found in nigral neurons may be synthesized locally within the neurons or can be acquired by retrograde transport from the striatal neurons (40). The melanized dopamine-containing neurons of the substantia nigra show a greater loss of BDNF than the nonmelanized cells (41,42). BDNF prevents nigral degeneration induced in MPTP models of PD (43). GDNF, a neurotrophic factor belonging to the TGF-b superfamily, and its receptors, Ret and GDNF-a1, are expressed highly in the dopaminergic neurons of the substantia nigra (8,33). GDNF concentrations are decreased in the nigral neurons in PD (44). When injected in the striatum, GDNF is selectively transported retrogradely to nigral dopami- nergic neurons (40). GDNF, like BDNF, protects mesencephalic dopamine neurons from 6-OHDA and MPTP toxicity and improves motor functions in these models of PD (45,46). However, the molecular pathways that mediate neuroprotective effects of these two neurotrophic factors may be different (47). Among the neurokines, an increased level of interleukin-6 (IL-6) has been demonstrated in the striatum. Many cytokines, which are traditionally recognized to play anti- inflammatory roles outside the brain, have now been recognized to be expressed in the brain (48) in response to tissue injury or inflammation (e. Some of these cytokines may be neuroprotective and others apoptotic. In this regard, levels of TNF-a, TGF-a, TGF-b, IL-1b, IL- 4, and IL-6 are elevated in the striatum in PD. This has led to the hypothesis that the pathogenesis of PD may be the result of an imbalance between the actions of the antiapoptotic neurotrophic factors and the proapoptotic factors (50,51). Several neurotrophic factors have been recognized to be expressed by nonneural cells.

The muscles that are tightening with the flexion extension movement of the knee are the semimembranosus and semitendinosus trileptal 600mg without a prescription, whereas the muscle belly that does not tighten with this maneuver is the adductor magnus. The interval is opened between these two muscles until the femur is palpated. As soon as the femur is palpated, the finger should be turned around and the space between the linea aspera inserting into the fe- mur and the femoral shaft carefully palpated to identify the sciatic nerve. When the sciatic nerve has been identified definitely, it feels like an overcooked, soft noodle and does not tighten like the hard tendon superficial to it (Figure S3. The finger is placed past the nerve, turned 180°, and all the muscles superficial to the nerve are swept up. These muscles should include the semitendinosus, semimembranosus, and long head of the biceps (Figure S3. A right-angle clamp is then placed around this mus- cle mass and the finger is removed (Figure S3. A retractor is placed along the medial wound and another right- angle retractor is used to pull up bunches of this muscle. Care is taken to ensure that it is red muscle that is being transected with elec- trocautery. All white structures should be checked with a battery- powered nerve stimulator and the child must be nonparalyzed to make sure that there is no damage to the sciatic nerve or inadvertent cutting of the sciatic nerve. In general, the biceps femoris muscle comes up first, followed by the semitendinosus, and then the hard, white tendon that remains is the semimembranosus, which is most easily confused with the sciatic nerve. There will be some muscle fibers attached to the semimembranosus and it does have the ap- pearance of a tendon.
